Puncture laser decompression of the intervertebral disks as a method of treatment vertebrogen radiculopathy

Puncture laser surgery degeneratinyh intervertebral discs of the lumbar spine were performed 180 patients with vertebrogenic radikulpatiey. Relief of pain and regression of neurological disorders were obtained in 67,8% cases. Restoration of neurological symptoms was noted in all patients.

On some late vascular complications of diabetes

One of the most important complication of diabetes mellitus is the development of ischemia of a myocardium. The high level lipids in blood and high blood pressure are two main factors of ischemia of a myocardium and atherosclerosis. The metabolic breach at the diabetes mellitus is very various: tissue gipoksy, hormone disbalance, metabolic acidor, angiopatia and electrolytic breach and etc. On condition of ischemia myocardium beginning anaerobic exchange, oxidizing phosphoresention abruptiy siowing down, the contents of loctats is raising, correlation of loctate / piruvait is growing, raising the consumption of the glucose by ischemia myocardium. So the deginning of ischemia myocardium at the diabetes mellitus represents disintegration metabolic and hormone intercommunications with the accent on breach of carbohydratic and fatty exchange.

Dynamics of cognitive and affective disorders at the patients in the acute period of the cerebral stroke in dependence on the sex

The 27 patients with an ischemic stroke, 10 men, 17 women, were investigated. The index of depression at women was expressed less than at men, and the index of anxiety at women was more in the acute period of the ischemic stroke. The significant improvement of the cognitive functions was observed in both groups after treatment (р<0,1). Parameters of depression and anxiety were decreased in both groups, but the men achieved the best results.

The regional cold perfusion in preventive maintenance of the reperfusion syndrome

Comparison and efficiency studying of local perfusion and local hypothermia on not purebred dogs with control group with model of an acute ischemia of extremities with subsequent reperfusion is performed. The best results in comparison with local hypothermia are received at application of the offered technique of the regional cold perfusion. Efficiency of both techniques in comparison with control group is proved.

The sex- and age-specific clinical features and the cognitive deficit in the inhabitants of the far north with the chronic cerebral ischemia

The 47 patients with cerebrovascular diseases in neurological separation SE The 47 patients with cerebrovascular diseases in neurological dranch are surveyed. The prevalence of men was marked in all groups of patients . The middle age of patients has made 64,7±10,9 years. The men age is below than the age of women statistically significantly. At men the reduction of complaints was significantly after a therapy (р?0,05). The decrease of insomnia indexes was registered. At Indicators of depression after treatment have decreased, and indicators of situational anxiety have grown. Thus indicators of anxiety were more low at women, than at men after treatment . The decrease of depression is more expressed at patients younger than 60 years. The overall performance has grown in group till 60 years more considerably, than at persons of older age.

Some problems related to the arterial-venous fistulae in hemodialized patients

With the aim to improve patency of arterial-venous fistulae life in patients with end stage kidney insufficiency we studied the results and problems got in such patients who underwent operation for arterial-venous fistulae creation. Our study includes 101 patients were vascular access for hemodialysis was necessary. The period of the study is May 2005-September 2009.as it will come out by the study we need to have a better control of the disease process and improve the cooperation between nephrology and surgery departments.

Arterial reconstruction in the aorto-iliac segment. Is the choice of vascular prostheses significant?

Aortofemoral reconstructions still remain the gold standard for assessment of early and late results in the treatment of patients with various obliterative arteriopathies.

The most common and serious complications after such operations are acute thrombosis and hemorrhage, as well as earlier infection of the prosthesis.

Having used different artificial vascular prostheses for reconstruction in the aorto-iliac segment with 350 patients, authors conclude that indicated complications in the early postoperative period do not depend on the type of prostheses and the material from which they were produced.

The decisive factors for the success of the operation still remain the maximum atraumatic technique, shortening the time of surgery, selection of appropriate antibiotics and the rational method of application, modern approach to intra-and postoperative anticoagulation therapy.

Along with the preventive use of muscle plastic in femoral access in infection, the authors along with the removal of the prosthesis of the prosthesis have also successfully applied omentoplastic.

Chronic ischemia of brain and cognitivnye disorders at inhabitants of the north territories

All cases of the hospitalization of patients with cerebrovascular diseases in a republican neurological separation from 1998 to 2008 are analyzed, 47 patients are inspected. Meaningful prevalence of women in all groups of patients is marked. The men age is below than the age of women statistically significantly. The disease began more early at the patients, resident in the districts of the far north. The decline of indexes of insomnia is exposed after the course of therapy. The indexes of depression decreased, and the indexes of situational anxiety grew. Along with the positive dynamics of time of work and psychical stability, the indexes of brief memory did not change.

Physical rehabilitation of patients recovering from spinal-spinal trauma

A comprehensive approach to the rehabilitation of patients with complicated trauma of the thoracic spine. To restore the lost functions of support and movement after injury of the thoracic spinal cord in a methodical plan was allocated to three main stages of motor rehabilitation, consistently critical task of restoring the methods and means are adequate, as the motor areas of the patient. We in the organizational plan proposed a model watershed-course of rehabilitation, providing the tasks of each stage of physical rehabilitation through inpatient treatment and periods of self-training at home. The proposed method of physical rehabilitation in the late period of traumatic disease of the thoracic spinal cord allows optimal use of the means of physical culture for possible recovery of lost functions of support and movement, promotes the acquisition of skills of persistent self-study patients in subsequent periods of his life.

Conservative treatment of discogenic neurological manifestations of degenerative-dystrophic diseases of the spine

Osteochondrosis of the lumbar spine is the most common degenerative-dystrophic diseases of the musculoskeletal system in those middle-aged and elderly. At present, more effective conservative treatment of patients with degenerative-dystrophic diseases of the lumbosacral spine, complicated by herniated intervertebral discs in the outpatient phase is the most important problem of the recovery period of the disease.

Definition of the cerebrolysin-dependent activation of the B-cells as the method of laboratory diagnostic of the neurological signs of rheumatoid arthritis

The problem of rheumatoid arthritis (RA) keeps actuality. Acuteness of the question of the nervous system (NS) lesions during RA new verification methods is still high. The goal of the work is to prove possibility of usage of definition of cerebrolysin-dependent activation of the B-cells (CDABC) as the method of diagnostic for neurological signs of RA. 64 patients with neurological signs of RA were examined (main group). 3 groups of people were investigated for compare. The 1st one was consisted of 42 patients with RA but without of neurological signs. The 2nd group – 30 patients with affections of the peripheral nervous system of non-autoimmune genesis. The 3rd group united 30 patients with CNS pathology of non-autoimmune genesis. 30 healthy people were investigated as the control. Definition of CDABC was conducted in the monolayer of lymphocytes with the help of quantity cytopfuorometry. The definition of CDABC indexes was proved to be the method of diagnostic for neurological signs of RA with high sensitiveness (90.6%) and specificity (100%). The method of definition of present index is simple, quick and chip and those characteristics can be the basis for usage of that method for screening of neurological pathology among patients with RA.

The clinical and epidemiological characteristic of muscular dystrophy in Republic Komi

The data about 57 patients was studied. The muscular dystrophy debuted at young age at inhabitants of Komi. The age structure of a pathology was characterised by reduction of a share of patients of older age. In gender structure the men were prevalenced insignificantly. The activity of muscular disintegration was higher at early stages of disease, and was decreased with the years. Maintenance CPK at men was exceeded one at women significantly, despite approximately equal duration of disease

Reliability of clinical tests in developmental dysplasia of the hip

Background: Diagnosis of DDH is still a challenge for the health system in our country. Screening should begin with clinical tests and their technique should be well known and performed not only by orthopedic surgeons but also other specialists like pediatricians, family physicians, obstetricians and nurses. Material and method: In 111 children (143 hips) already diagnosed and treated for x-ray confirmed DDH, we retrospectively analyzed if the Ortolani/Barlow and Limitation of Abduction tests had been reported to be positive in their clinical records in order to evaluate the reliability of those tests. We also compared the rate of positive results, when the orthopedic surgeon or other specialists performed each test. Results: Records of Ortolani/Barlow test showed that 34.9% of hips (50 out of 143) were reported as positive from the other specialists and 64.3% of them (84 from 143) when the Orthopedic surgeon was the examiner(p=0.014). No significant differences were observed when the Limitation of Abduction was analyzed. Both groups of examiners identified about 67% of the 143 affected hips. (92 hips from the other doctors versus 98 form the Orthopedic Surgeon, p=NS). Conclusion: Clinical tests resulted very useful for the early diagnosis of DDH. The limitation of Abduction test is more easily performed and simple than the Ortolani/Barlow maneuvers and should be used as the primarily screening tool from the specialists outside the orthopedic field.

Justification for the development and implementation of the complex non-medicament methods in men with hypertension and adjustment disorders

The article reports the results of assessment of 255 male patients with hypertension. The anxiety was detected in 71,37% of the cases (182 men), subsequent study found adjustment disorders in 93 of them (36,47% from all patients and 51,10% from patients with anxiety. Using of the patented comlex non-medicamental method in treament men with hypertension and adjustment disorders demonstrated high efficiency.

Repeated cerebral stroke at inhabitants of the north territories and risk factors

The information about the 1224 cases of ischemic stroke were studied. The patients with the repeated ischemic stroke made the 21,5% of this group. Among the studied people the equal number of men and women were established. Women were more senior than men (r<0,001). The lethality of patients with the repeated stroke was significantly higher (r<0,001). The hypertension prevails among the risk factors of the patients with the repeated strokes (r<0,001). It is not marked difference on frequency of the repeated strokes depending of the factors of risk taken into account in Register and in all group of patients with a stroke practically. The significant increase of frequency of the repeated ischemic strokes is exposed only at persons with the myocardial infarction or other diseases of heart in anamnesis.
